Please use this hotel reservation form and either mail or fax it directly to the Days Inn, Santa Clara.
It is vital that hotel reservations reach the Days Inn Santa Clara by May 13,1998 to have the special Workshop rates.
The rates will also be available for Arrival on June 3, 1998 Departure on June 7, 1998.
